INTRODUCTION: Avascular necrosis (AVN) of the head of the femur is a potentially crippling disease which mainly affects young adults. Treatment options differs, according to the stage of necrosis. Although treatment by exposure to hyperbaric oxygen (HBO) is reported as being beneficial, there has been few studies about HBO treatment. In this study early term results of HBO therapy as a treatment for Ficat stage I-IIA avascular necrosis of the femoral head were presented.
METHODS: Four femoral head avascular necrosis (one bilateral involvemet) of three patients with mean age 43.6 were impicated to the study. Daily HBO therapy was given for 40 days to each patient All patients were evaluated according to Harris hip score before the therapy and after 1 year follow up.
RESULTS: As results,while median Harris hip score(HHS) was 28 before the HBO treatment (ranged between 19-34) with SD 7.93, Average Harris hip score was increased to 76.66 (ranged between 69- 89 with SD 10.785, after 1 year follw up. The difference between before and after HHS values were statistically significant. (p<0.001)
DISCUSSION AND CONCLUSION: Although the limited number of patients were impicated to the study, hyperbaric oxygen is effective and alternative method in the treatment of early stage without collaps, avascular necrosis of the head of the femur.

GİRİŞ ve AMAÇ: Femur başı avasküler nekrozu (AVN) özellikle genç populasyonda sakatlığa neden olma potansiyeline sahip bir hastalıktır. Tedavi seçenekleri hastalığın hangi evrede bulunduğuna göre farklılık gösterir. Femrur başı AVN tedavisinde farklı cerrahi prosedürler tarif edilmiş olsa da hiperbarik oksijen tedavisi (HBO) ile ilgili literatürde kısıtlı sayıda çalışma bulunmaktadır. Çalışmamızda Ficat Evre I-IIA femur başı AVN’lu olgularda uygulanan hiperbarik oksijen tedavisinin erken dönem sonuçları bildirilmektedir.
YÖNTEM ve GEREÇLER: Çalışmamıza ortalama yaşları 43.6 olan 3 hasta, biri bilateral olmak üzere toplam 4 femur başı AVN’lu olgu dahil edilmiştir. Her üç olguya da 40’ar seans HBO tedavisi uygulanmıştır. Hastalar, HBO tedavisi öncesi ve tedavi sonrası 1. yıl takiplerinde klinik olarak Harris Fonksiyonel Kalça Değerlendirme Skorlaması (HKS) ile değerlendirilmişlerdir.
BULGULAR: Sonuç olarak, tedavi öncesi HKS 19-34 (ort. 28) ve SD 7.93 iken, 1. yıl takiplerinde tedavi sonrası HKS 69-89 (ort. 76.66) ve SD 10.785 olup, iki değer arasında istatiktiksel olarak anlamlı fark olduğu tespit edilmiştir (p<0.001).
TARTIŞMA ve SONUÇ: Bu çalışma az hasta sayısı ile gerçekleştirilmiş olmasına karşın, HBO tedavisinin erken evre, kollapsın gelişmediği femur başı AVN’lu olgularda tedavi alternatifleri arasında düşünülmesi gereken bir yöntem olduğunu göstermiştir.
