This study has been carried out on 17 newborns who were initially diagnosed as neonatal sepsis in the NICU (Newborn Intensive Care Unit) as well as 17 healthy newborns at the Medical Faculty Hospital of Gaziantep University to investigate the role of serum TNF-α concentration in the early diagnosis of neonatal sepsis and its relation with the prognosis. The clinical and hematological criteria of Silveira et al. (22) are used in the diagnosis of sepsis. Sepsis was diagnosed with at least 3 positive clinical signs and two positive inflammatory laboratory findings meeting these criteria or positive hemoculture. In the study and control groups plasma samples were taken only for once. Serum TNF-α concentrations of the newborns with sepsis (9.25±14.13 pg/ml) were not significantly higher than the control group (7.14±4.31 pg/mL) (p>0.05). Serum TNF-α concentrations of the patients who died were not significantly higher than the survivors (p>0.05). These data suggest that TNF-α, an important macrophage derived member of the cytokine family, hasn’t an important role in the early diagnosis of neonatal sepsis; and that there is a close relationship between its plasma levels and mortality rates. Gaziantep Tıp Fakültesi Çocuk Sağlığı ve Hastalıkları Kliniği Yenidoğan Servisinde yapılan bu çalışmada, sepsis öntanılı 17 hasta ve 17 sağlıklı toplam 34 yenidoğanın serum örneklerinde TNF-α konsantrasyonları araştırıldı. Sepsis tanısında Silveira ve ark.’nın (22) kriterleri kullanıldı. Sepsis tanısı bu kriterlerden en az 3 klinik bulgu ve inflamatuvar fenomenlerden ise en az 2 laboratuvar bulgusunun pozitif olması ya da kan kültürünün pozitif olması ile konuldu. Çalışma ve kontrol grubundan bir kez analiz için kan alındı. Sepsisli yenidoğanların serum TNF-α düzeylerinin (9.25±14.13 pg/mL), kontrol grubunu oluşturan sağlıklı infantlara göre (7.14±4.31 pg/ml) anlamlı derecede yüksek olmadığı belirlendi (p>0.05). Ölen hastaların serum TNF-α konsantrasyonlarının sağ kalanlara göre anlamlı derecede yüksek olmadığı belirlendi (p>0.05). Elde ettiğimiz sonuçlar, makrofajlar tarafından sentezlenen ve sitokin ailesinin önemli bir üyesi olan TNF-α’nın, yenidoğan sepsisi erken tanısında önemli bir role sahip olmadığını ve prognozla ilişkisinin bulunduğunu göstermektedir Anahtar Kelimeler: Tümör nekroz faktörü-alfa, yenidoğan sepsisi, prognoz
