Osteoid osteomas in the phalanges of the hand often present with atypical clinical and radiologic characteristics. These unusual presentations may cause a delay in diagnosis and treatment. In this case, one year after a nonspecific trauma and with a history of pain of four monthly only, an osteoid osteoma of the proximal phalanx of the left third finger was treated in a 21-year-old man.
