Moya Moya disease is a chronic vasculopathy which causes unilateral or generaly bilateral progressive oclusion of internal carotid arteries resposible for circulation of anterior part of cerebrum. This disease cause cerebrovascular insuffiency and that incuffiencies multiple anastamosis and collaterals between internal and external carotid arteries. In this article, we presented this very rare disease occuring in a two year nine months old boy with a down syndrome who has localized convulsions. Keywords: Moya Moya disease, Down syndrome
