Tongue tremor can be seen in patients with essential tremor, but when it is isolated, it can be due to tumors of the brain stem, Wilson’s disease, multiple sclerosis, hyperthyroidism or side effect of a drug. Other movement disorders of the tongue diseases (fasciculation, myokimia, dyskinesia) should be considered in the differential diagnosis and the patients should be investigated in detail. Propranolol, primidone, and clonazepam are preferred in therapy and most of the time response to therapy can only be achieved at higher doses. In this case, we are presenting a patient diagnosed as isolated tongue tremor, who had complaints of tongue tremor, eating and speech difficulties whose complaints resolved with low-dose propranolol. Dil tremoru esansiyel tremoru olan hastalarda görülebildiği gibi izole olduğunda beyin sapı tümörleri, Wilson Hastalığı, hipertiroidizm ve multipl skleroz gibi hastalıklara veya ilaç kullanımına bağlı olarak da ortaya çıkabilir. Ayırıcı tanıda dilde görülen diğer hareket bozuklukları (fasikülasyon, miyokimi, diskinezi) düşünülerek hastalar ayrıntılı olarak araştırılmaktadır. Tedavide propranolol, primidon ve klonazepam tercih edilmekte olup, çoğu zaman yüksek dozlarda etkinlik sağlanabilmektedir. Bu olgu sunumunda dilinde titreme, yeme ve konuşma güçlüğü çeken ve yakınmaları düşük doz propranololle düzelen izole dil tremoru tanısı koyduğumuz bir hastayı sunmaktayız.
