Hepatocellular carcinoma (HCC) is the most common primary malignant tumor in liver and it is one of the most common cancers worldwide. The most important risk factors for HCC are cirrhosis, viral hepatitis, alcohol abuse and metabolic liver disease. HCC presents with hepatic involvement in majority and as metastatic disease in minority of patients. HCC commonly makes metastases to lungs, lymph nodes, adrenal gland, whereas bone involvement is rare. Bone metastases of HCC, usually do not give symptom unless any pathological fracture happens or before they reach great dimensions. We present a case of HCC confirmed by the biopsy from the iliac bone which is a rare metastatic region of HCC. Keywords: Hepatocellular carcinoma, cirrhosis, bone metastasis
