INTRODUCTION: When the vaginal ecosystem becomes unbalanced, as in the case of bacterial vaginosis, the risk of postoperative infection increases noticeably. In this study, we aimed to investigate the change of vaginal flora under antibiotic prophylaxis after hysterectomy and radical operations.
METHODS: In this study, sixty-four patients who had undergone hysterectomy for a variety of indications between 2011 March and 2011 September in gynecology clinics of Istanbul Medeniyet University Goztepe Research and Training Hospital Department of Obstetrics and Gynecology were evaluated retrospectively. All patients had been treated with vaginal douche with % 10 povidone iodine and antibiotic prophylaxis before surgery. We obtained samples for culture from vaginal stump with cotton swab before all patients were discharged. Samples were sent to bacteriology laboratory of our hospital.
RESULTS: The most common colonized bacteria in 31 cultures was E.coli and eight strains were extended spectrum beta lactamase producing E.coli. Only two patients had wound infections and from vaginal and wound cultures of these patients, beta lactamase producing E.coli was colonized.
DISCUSSION AND CONCLUSION: In this study, we emphasize the fact that in all cases, infection control should be done in consideration of postoperative physical examination and bacteriological laboratory tests, despite preoperative antibiotic prophylaxis.

GİRİŞ ve AMAÇ: Vajinal ekosistem dengesi bozulduğu zaman, bakteriyel vajinozda olduğu gibi, postoperatif infeksiyon riskinde belirgin derecede artış görülmektedir. Bu çalışmada histerektomi ve radikal operasyon sonrası antibiyotik profilaksisinin vajinal flora üzerine etkisini araştırmayı amaçladık.
YÖNTEM ve GEREÇLER: Bu çalışmada 2011 Mart-2011 Eylül arası İstanbul Medeniyet Üniversitesi Göztepe Eğitim ve Araştırma Hastanesi Kadın Hastalıkları ve Doğum Kliniği'nde çeşitli endikasyonlarla histerektomi olan 64 hastayı retrospektif araştırma kapsamında inceledik. Bütün hastalara cerrahi öncesi antibiyotik profilaksisi ve % 10’luk povidon iyot ile vajinal duş uygulanmıştı. Bütün hastalardan eksterne edilmeden önce vajinal kültür alınmıştı.
BULGULAR: En çok kolonize olan bakteri 31 hastadan izole edilen E.coli idi ve sekiz tanesi b-laktamaz yapan E.coli suşuydu.Yalnızca iki hastada yara infeksiyonu vardı. Yara yeri ve vajinal kültürden b-laktamaz üreten E.coli izole edildi.
TARTIŞMA ve SONUÇ: Bu çalışmada antibiyotik profilaksisine rağmen, infeksiyon kontrolünün fizik muayene ve laboratuvar testleri ile kontrol edilmesi gerektiği sonucuna vardık.
