A 42 years old man was referred to our clinic for evalution of acute alteration in clinical status. His medical history included mitral and aortic valve operation due to rheumatic valvuler disease 7 years ago and inadequate treatment for brucellosis 7 months ago. Sistolic murmur at middle left border of sternum was heart and tachycardia was observed on physical examination. The patient was hospitalised. Administiration of empric vancomycin and gentamycin combination therapy had been initiated to treat prostetic valve endocardity. After several days in the hospital the clinical status persisted.Vegetations on prostetic aortic valve were seen in transesophageal cardiac echo and brucella tube agglutination test was positive at a titer 1/640. The patient was diagnosed as brucella endocarditis and was treated rifampicin and doxycline. Keywords: Brucella, endocarditis
