Brucella endocarditis is a disease with high morbidity and mortality. The congenital or acquired primary pathology frequently facilitates the invasion of the active microorganism into the cardiac tissues. In our case, 43 years old male patient having high fever and dyspnea showed severe aortic insufficiency, moderate aortic stenosis and severe mitral insufficiency in his echocardiography. Rose-Bengal and Brucella tube agglutination was found positive. Antibiotics therapy was administered during preoperative three weeks. In operation, brucella endocarditis was diagnosed on the aortic valve with rheumatic involvement. Aortic valve was replaced by mechanical prosthesis and mitral valve was repaired. In this study we aimed to emphasize the importance of valvular repair in an active brucella endocarditis. Keywords: Brucella endocarditis, mitral valve repair
