Myasthenia gravis is an autoimmune disease, resulting from the production of antibodies against the acetylcholine receptors of the neuromuscular endplate. These antibodies reduce the active receptors, brought about either by functional block of the receptors, by increased rate of receptor degration, or by complement-mediated lysis. Myasthenic patients are special for anaesthesia. The myasthenic patient may demonstrate an abnormal neuromuscular response to both depolarizing and nondepolarizing muscle relaxans. Volatil induction and maintenance was successfully employed for patients without myo-pathy. Inhalational agents can be used for anaesthetic induction of myasthenics with the advantage of avoiding the use of neuromuscular blocking agents.
In this study, we present a myasthenic case, for her umblical hernia operation in which sevoflurane was used in induction and in maintainence of anaesthesia as a single agent.
